For referrers: 

We accept referrals from allied health, medical and non-medical professionals. In order to best assist your client with their problem please feel free to complete the below referral form and sent this to reception@evolvingpain.com.au

We will then reach out to your client and provide them with the information about what to expect throughout their experience with Evolving Pain. We will provide the option of discussing any clinical concerns with our director, Rachael Sheat before commencement.

 

With their consent, we will then collaborate and involve you in correspondence of their time with Evolving Pain.

If you are unsure about whether Pain-specific Physiotherapy is suitable for your client please use the below checklist to determine suitability for our clinic (please note we also see acute musculoskeletal injuries).

For prospective clients: Do you need a referral to come to Evolving Pain?: 

It is important to know that you do not officially require a referral to see us, however any information that you feel would be valuable to share with us will assist us in supporting you, and that includes correspondence from your health and medical professionals.

If you have information to pass onto us please email it through to reception@evolvingpain.com.au, or if you are not comfortable emailing your personal information please feel free to drop this into the clinic before your initial appointment.

Cancellation and Non-Attendance Policy

 

As a private pain clinic we’ve had to give this one some serious consideration, as chronic pain can come along with a myriad of other health concerns. Our clinicians are super sensitive to this, however we also need to look after our accessibility to other members of our pain community. 

 

For this reason we ask that you provide us at least 24 hours if you need to cancel your appointment, to allow for someone else in need to jump in your place. 

 

With respect to our clinicians and our fellow pain community we have agreed that cancellations less than 24 hours notice will be charged 50% of the consultation fee, and cancellations with less than 12 hours notice, or missed appointments will be charged 100% of the appointment fee

 

On the odd occasion if you are unwell and cannot provide this notice period please get checked out, and will waive the fee upon presentation of a medical certificate. 

 

We don’t want to charge you a cancellation fee, so please always remember that we work flexibility to allow for a change in your circumstances. If you are not up for visiting the clinic and you are having a bad day, notify us and we are always happy to switch to Telehealth or even a phone call if you prefer. 

 

It is important to be mindful that in most instances third parties do not fund late cancel/missed appointment charges, so regretfully we will need to pass this directly onto you for prompt payment.
